First Visit

Your initial appointment will involve a consultation where we will discuss your diagnosis and treatment options. In some cases, surgery may be performed on the same day as your consultation. However, if you have a complex medical history or treatment plan, we may need to schedule a follow-up appointment for your procedure.

To help us prepare for your consultation, please bring the following information:

• Your surgical referral slip and any relevant x-rays, if applicable.
• A list of medications you are currently taking.
• Completed forms for any medical or dental insurance you may have. This will expedite the process and assist us in handling any claims.

IMPORTANT: All patients under 18 must be accompanied by a parent or guardian during the consultation.

Patient Registration

For patients undergoing IV anesthesia, a preoperative consultation and physical examination are required. Please do not eat or drink anything for eight hours before your surgery, and ensure you have an adult to drive you home afterward.

If you have any medical conditions that may be relevant to your surgery—such as diabetes, high blood pressure, artificial heart valves or joints, or a history of rheumatic fever—please inform our office beforehand. Additionally, let us know if you are currently taking any medications, including heart medications, aspirin, or anticoagulants.

X-Rays

If your dentist or physician has previously taken x-rays, please request that they send them to our office. If time is limited, you may pick up the X-rays and bring them with you. Should additional X-rays be necessary, they can be taken at our facility.
